State Program Descriptions

According to the Minnesota Department of Human Services:

 

Minnesotans with disabilities or chronic illnesses who need certain levels of care may qualify for the state's home and community-based waiver programs.

 

Medicaid home and community-based service (HCBS) waivers afford states the flexibility to develop and implement creative alternatives to placing Medicaid-eligible people in hospitals, nursing facilities or Intermediate Care Facilities for Persons with Developmental Disabilities (ICFs/DD). HCBS waivers allow states to put together various service options that are not available under regular Medical Assistance. These service options are available to people in addition to services covered by Medical Assistance. Generally, these services are targeted to people with specific needs or diagnosis.

 

Minnesota has 6 waiver programs; Alternative Care (AC), Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI), Developmental Disabilities (DD), and the Elderly Waiver (EW). One of the service delivery options for waiver participants is Consumer Directed Community Supports (CDCS), which gives the participant the flexibility to design their own plan for the services and supports they need. 

As a waiver recipient selecting the CDCS service delivery option, participants can write their own plan, or hire a support planner to assist them.

 

Support Planning

A support planner assists Medicaid Waiver recipients in creating a person-centered community support plan to receive services through the Consumer Directed Community Supports (CDCS) program. 

In addition to writing the plan, a support planner can:​

  • Provide information on and assist in selecting a Fiscal Management Service

  • Find services and supports

  • Make arrangements for obtaining services and supports

  • Help solve problems that may occur

  • Help monitor the community support plan

Our staff have over 20 years of experience working with disabled individuals and their families. All of us either qualify as disabled ourselves or have a family member with a disability. This combination of professional and personal experience ensures that all of our services are provided with a unique perspective.

We work with ALL Minnesota counties, utilizing video or teleconferencing to reach rural areas that have little or no access to support planners.

We offer culturally specific services and are able to provide services in Spanish.

We believe that it is important for EVERYONE to have access to high quality support planning. We charge an hourly rate, but all plans have maximum billable rate. This eliminates the guesswork involved in having solely an hourly fee, as you will never be billed more than the maximum rate for an initial or renewal plan.  It ensures that you get access to the same quality plan, regardless of how much time it takes. 

 

All of the services are billed through your Fiscal Management Service as part of your Community Support Plan. We will only provide services as identified in a work agreement that you approve.
