Billing Information

Minnesota Disability Support Alternatives is an approved contracted Community Partner for Pre-ETS services with State of Minnesota Vocational Rehabilitation Services through June 2024.

We request that for our Self-Advocacy Training and Support Program, to please authorize the following:
Type: Pre-ETS Intake: 1 each
Type: Pre-ETS Workplace Readiness Training: Employee Development Services, Job Seeking Skills Training, Independent Living Services. 10 hours group, 6 hours individual.

If students elect to add additional individual coaching meetings beyond the session, those hours would be added to the authorization under individual hours. Please note that additional individual coaching meetings depend on student need and facilitator availability.
