Waiver Billing Details
TRADITIONAL WAIVER BILLING
As units are utilized, we will bill the waiver on a monthly basis. The information below is only for the session itself. If students elect to add additional individual coaching meetings beyond the session, an updated service authorization including those units will be requested. Please note that additional individual coaching meetings depend on student need and facilitator availability.

BI, CAC, DD and CADI Waivers are billed under “Family Training and Counseling” under the following service code: Family Training S5110 (in person) S5110 U4 (virtual)
Under S5110/S5110 U4 we ask that you authorize
48 units (12 hours of time) at $10.80/unit (average rate for group/individual)
Date Range: Should include at least 3 month timeframe to accommodate missed classes or meetings that may need to be rescheduled

*MNDSA must receive Service Authorization prior to the person joining any class.
* Please ensure proper authorization to share information with MNDSA is obtained by the appropriate
CDCS Community Support Plans are written by the participant or their support planner. The final decision as to how to include billing information in their plan may vary based upon their case manager or FMS requirements. However, it is suggested that along with adding the required goals related to the participant's MnChoices, the following breakdown of cost could be included:
Self-Advocacy Training and Support Program Group Class, MNDSA - 8 hours at $30.00 per hour
Self-Advocacy Training and Support Program Individual Coaching Meetings, MNDSA - 4 hours at $75.00 per hour.
If students elect to add additional individual coaching meetings beyond the session, those hours would be added to the Individual Coaching Meeting line. Please note that additional individual coaching meetings depend on student need and facilitator availability.
