At MNDSA, we believe that you are the expert on your best life. Our programs provide the information, planning, and peer connection you need to live the life you envision — in the community of your choice. We serve all 87 Minnesota counties, with virtual and teleconference options available statewide.

Minnesota Home and Community Based (HCBS) waivers can be confusing — especially if you're new to the system. Our team helps you understand what waivers are available, whether you may qualify, and how to begin the process of accessing services through CDCS or other funding streams. We also offer easy-to-follow guides to county services in four languages.

Waiver Navigation

MNDSA provides information and coaching on education options for youth with disabilities — options that accommodate specific needs and ensure academic success and workforce readiness. Whether you're exploring home education, public or private education, or need guidance on available accommodations and supports, we can help.

Disability Education Support

The B.R.I.D.G.E. Program connects individuals with disabilities and their caregivers to personalized resources through meaningful mentorship and advocacy. Our goal is to empower you to define and pursue your ideal life — providing guidance, encouragement, and the tools to navigate systems, build confidence, and advocate for your unique goals and values. The B.R.I.D.G.E. Program is funded by the Home and Community Based Services (HCBS) Connectors grant through the Minnesota Department of Human Services. Guides available in English, Oromo, Somali, and Spanish.

B.R.I.D.G.E. Program

A support planner helps Medicaid Waiver recipients create a person-centered Community Support Plan to receive services through the Consumer Directed Community Supports (CDCS) program. In addition to writing the plan, your support planner can help you select a Fiscal Management Service, locate and arrange services and supports, troubleshoot problems as they arise, and monitor your plan over time.  Our staff have over 20 years of experience working with disabled individuals and their families. All of us either qualify as disabled ourselves or have a family member with a disability. We work across all 87 Minnesota counties, utilizing video and teleconferencing to reach rural areas with limited access to support planners.
